Acetylsalicylic acid + Clozapine

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Additive bleeding risk due to aspirin’s antiplatelet effect in the setting of potential clozapine-associated thrombocytopenia; increased risk of mucosal/GI and other bleeding.

Mechanism of action

Pharmacodynamic: Aspirin irreversibly inhibits platelet aggregation (COX-1). Clozapine can cause hematologic abnormalities including thrombocytopenia; combined effects can impair hemostasis. No clinically significant pharmacokinetic interaction expected.

Clinical management

Assess baseline and periodic CBC with platelets; counsel on bleeding signs (bruising, melena, epistaxis). Use lowest effective aspirin dose; consider PPI gastroprotection if GI bleed risk is elevated. Avoid or hold aspirin if platelets <100,000/µL, active bleeding, or high bleeding risk; consider alternatives.
