Additive antihypertensive effects may increase risk of symptomatic hypotension (dizziness, syncope), especially at initiation or dose escalation.
Mechanism of action
Pharmacodynamic: arteriolar vasodilation from calcium-channel blockade plus RAAS inhibition from ACE inhibitor → enhanced BP lowering.

Monitor BP and orthostatic symptoms during initiation and titration; consider starting lower doses in elderly/volume-depleted patients; adjust doses if symptomatic hypotension occurs.
