Overview
High-dose salicylates may blunt the antihypertensive effect of beta-blockers and promote fluid retention; effect minimal with low-dose antiplatelet aspirin.
Mechanism of action
Pharmacodynamic: inhibition of renal prostaglandin synthesis → decreased renal blood flow, sodium/water retention → reduced BP-lowering effect.
Clinical management
Prefer low-dose aspirin when used for antiplatelet effect. If higher doses are needed, monitor BP and renal function; adjust antihypertensive regimen or choose alternative analgesic (e.g., acetaminophen) as appropriate.
