Drug interactions
aspirin + heparin
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Additive effects on hemostasis increase bleeding risk (GI, intracranial), especially with therapeutic heparin or higher aspirin doses.
Mechanism of action
Pharmacodynamic: aspirin irreversibly inhibits platelet aggregation (COX-1); heparin potentiates antithrombin to inhibit clotting factors → combined anticoagulant/antiplatelet effect.
Clinical management
Avoid combination unless strong indication (e.g., ACS/PCI). If used, prefer low-dose aspirin (75–100 mg/day); monitor for bleeding; check CBC (Hgb/Hct, platelets) and aPTT/anti-Xa as appropriate; consider GI protection; avoid additional antithrombotics.
