Overview

Severity: Major · Evidence level: High

Additive GI toxicity greatly increases risk of peptic ulcer and GI bleeding; corticosteroids can also alter salicylate levels.

Mechanism of action

Pharmacodynamic GI mucosal injury (both agents). Corticosteroids may increase renal clearance of salicylates; withdrawal may raise salicylate levels.

Clinical management

Avoid combination when possible. If needed, use lowest effective doses and co-prescribe a PPI; monitor for GI bleeding. Monitor salicylate toxicity when tapering steroids if on higher-dose aspirin.
