Drug interactions
aspirin + ibuprofen
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Ibuprofen can blunt aspirin’s antiplatelet effect and concomitant NSAID use increases GI bleeding risk.
Mechanism of action
Reversible COX-1 inhibition by ibuprofen competitively impedes aspirin’s irreversible platelet COX-1 acetylation; additive antiplatelet/NSAID mucosal injury elevates bleeding risk.
Clinical management
Avoid ibuprofen if aspirin is for cardioprotection. If necessary, dose ibuprofen ≥8 h before or ≥30 min after immediate-release aspirin; prefer acetaminophen. Use lowest NSAID dose, consider PPI in high-risk patients, and monitor for bleeding.
