Overview
NSAIDs (including aspirin) may blunt the antihypertensive effect of ACE inhibitors and increase risk of renal dysfunction and hyperkalemia; effect is dose-related.
Mechanism of action
Prostaglandin inhibition causes afferent arteriolar vasoconstriction and sodium/water retention, attenuating ACE inhibitor effects and reducing renal perfusion.
Clinical management
Avoid chronic NSAID use when possible. If aspirin is required, use the lowest effective dose; monitor BP, serum creatinine, and potassium at baseline and within 1–2 weeks after initiation or dose change.
