Overview
Additive antiplatelet effects may increase bleeding risk and prolong bleeding time when omega-3 fatty acids are used with aspirin.
Mechanism of action
Omega-3 fatty acids (EPA/DHA) can inhibit platelet aggregation and alter eicosanoid synthesis, producing additive antiplatelet/antithrombotic effects with aspirin.
Clinical management
Use cautiously; monitor for bleeding/bruising, epistaxis, GI bleeding. Avoid high-dose omega-3 (e.g., >3 g/day EPA+DHA) unless supervised. Consider holding omega-3 before invasive procedures.
