Mechanism of action
Pharmacodynamic: additive gastric mucosal injury and impaired hemostasis. Pharmacokinetic: corticosteroids may increase salicylate clearance; stopping steroids can raise salicylate levels.
Clinical management
Avoid or minimize combined use/duration; use lowest effective doses. Consider PPI gastroprotection in at‑risk patients; monitor for GI bleeding. If on higher-dose aspirin, watch for salicylate toxicity when tapering/stopping prednisone.
