Overview

Severity: Moderate · Evidence level: High

Salicylates/NSAIDs may blunt the natriuretic and antihypertensive effect of loop diuretics and increase risk of renal dysfunction, especially at higher aspirin doses.

Mechanism of action

NSAID-mediated inhibition of renal prostaglandin synthesis reduces renal blood flow and antagonizes loop diuretic natriuresis.

Clinical management

Avoid chronic/high-dose aspirin if possible; if needed, use lowest effective dose. Monitor BP, weight/edema, and renal function (SCr/BUN). Adjust diuretic dose based on response.
