Atorvastatin/Amlodipine + Atorvastatin/Ezetimibe

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Major · Evidence level: High

Therapeutic duplication of atorvastatin substantially increases statin exposure, raising risk of myopathy/rhabdomyolysis and hepatic enzyme elevations; ezetimibe coadministered with a statin may further increase myopathy risk.

Mechanism of action

Dose-dependent statin toxicity from combined atorvastatin products; possible small PK increase from amlodipine (weak CYP3A4 inhibition) is secondary.

Clinical management

Avoid concurrent use of two atorvastatin-containing products. Use a single atorvastatin regimen (with or without ezetimibe). If combined inadvertently, discontinue one product and assess for muscle symptoms; check CK and liver enzymes if indicated.
