Azithromycin + Cardiac Disease

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Patients with certain cardiac conditions (baseline QT prolongation, history of torsades, significant bradyarrhythmias, uncompensated heart failure) have increased risk of azithromycin-associated QT prolongation and sudden cardiac death.

Mechanism of action

Pharmacodynamic: azithromycin prolongs ventricular repolarization; underlying cardiac disease increases susceptibility to malignant arrhythmias.

Clinical management

Avoid azithromycin in patients with known QT prolongation, history of torsades, marked bradycardia, or uncompensated HF; select an alternative antibiotic when feasible. If use is necessary, obtain baseline ECG, correct electrolytes, and monitor for palpitations/syncope.
