Azithromycin + Omeprazole

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Omeprazole-associated hypomagnesemia (typically with prolonged PPI use) may increase susceptibility to azithromycin-induced QT prolongation and torsades de pointes. No meaningful pharmacokinetic interaction is expected.

Mechanism of action

Pharmacodynamic: Chronic PPI therapy can lower serum magnesium, which predisposes to ventricular arrhythmias with QT-prolonging agents like azithromycin; no significant CYP-mediated effect is expected.

Clinical management

If patient is on chronic omeprazole or has risk factors, check and correct magnesium and potassium before/during azithromycin therapy; consider ECG monitoring in high-risk patients; minimize other QT-prolonging drugs; consider alternative non–QT-prolonging antibiotic if baseline QTc is prolonged or significant arrhythmia risk exists; monitor magnesium periodically during long-term PPI therapy.
