For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Low

Risk of serotonin syndrome due to the serotonergic activity of amitriptyline when combined with rizatriptan; events are uncommon but clinically significant.

Mechanism of action

Pharmacodynamic: additive serotonergic effects (TCA serotonin reuptake inhibition with triptan 5-HT1B/1D agonism) increasing risk of serotonin toxicity.

Clinical management

Use together only if necessary; use the lowest effective rizatriptan dose; avoid adding other serotonergic agents or MAO inhibitors; educate patient on serotonin syndrome symptoms and monitor closely at initiation and dose changes; consider alternative acute migraine therapy if multiple serotonergic drugs are required.
