aspirin + eplerenone

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: High

NSAIDs, including high-dose aspirin, may worsen renal function and blunt antihypertensive effects; with potassium-sparing agents like eplerenone, hyperkalemia risk increases.

Mechanism of action

NSAID-induced reduction in renal prostaglandins decreases renal perfusion and renin/aldosterone activity, predisposing to hyperkalemia and reduced BP control.

Clinical management

Avoid chronic/high-dose NSAIDs if possible; if required, use lowest dose and monitor BP, SCr/eGFR, and serum potassium at baseline and after changes.
