Overview

Severity: Moderate · Evidence level: High

Aspirin (NSAID) may attenuate the antihypertensive effect of ACE inhibitors and increase risk of renal impairment, especially at higher doses.

Mechanism of action

Pharmacodynamic: inhibition of vasodilatory renal prostaglandins by aspirin opposes ACE inhibitor effects on renal hemodynamics; may reduce natriuresis.

Clinical management

Use the lowest effective aspirin dose; monitor BP, serum creatinine, and potassium at baseline and after initiation or dose changes; avoid dehydration and additional nephrotoxins.
