Drug interactions
atropine + epinephrine
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Additive tachycardia and proarrhythmic risk; atropine’s vagolytic effect can augment epinephrine-induced increases in heart rate and myocardial oxygen demand, potentially precipitating hypertension, ischemia, or arrhythmias.
Mechanism of action
Pharmacodynamic: Beta-1 adrenergic stimulation by epinephrine plus muscarinic antagonism by atropine reduces vagal tone and increases SA/AV node activity, leading to tachycardia and possible arrhythmias.
Clinical management
Avoid routine coadministration unless clinically indicated (e.g., resuscitation). If used, employ lowest effective doses and monitor HR, BP, and ECG/telemetry; use extra caution in ischemic heart disease or existing arrhythmias.
