Overview

Severity: Moderate · Evidence level: High

Additive risk of hyperkalemia and renal function decline when cyclosporine is used with an ACE inhibitor.

Mechanism of action

Cyclosporine can impair renal potassium excretion and renal function; ACE inhibition reduces aldosterone-mediated potassium excretion, increasing serum potassium and potentially lowering GFR.

Clinical management

Use with caution. Check serum potassium and creatinine at baseline and within 1–2 weeks of initiation or dose change; then periodically. Reduce dose or discontinue if K+ rises (>5.5 mmol/L) or creatinine increases >30%. Avoid added potassium or K-sparing agents.
