Overview
Fluoxetine can increase alprazolam exposure and enhance CNS depressant effects, leading to greater sedation and psychomotor impairment.
Mechanism of action
Fluoxetine/norfluoxetine inhibit CYP3A4, reducing alprazolam metabolism (pharmacokinetic) and both have CNS depressant effects (pharmacodynamic).
Clinical management
Use the lowest effective alprazolam dose; consider dose reduction and monitor for excess sedation, confusion, ataxia, and respiratory depression. Avoid or limit concomitant CNS depressants (e.g., alcohol, opioids). Reassess need for alprazolam after starting, stopping, or changing fluoxetine dose.
