Mécanisme d'action

Leptin analog that binds to and activates the human leptin receptor (ObR), a Class I cytokine receptor that signals through the JAK/STAT pathway, replacing deficient leptin in generalized lipodystrophy.

Posologie adulte habituelle (résumé de la notice)

Generalized lipodystrophy, males greater than 40 kg: start 2.5 mg subcutaneously once daily; adjust by 1.25 mg to 2.5 mg; maximum 10 mg/day

Résumé de la notice pour la première ligne de posologie uniquement ; les sections complètes sur la posologie figurent dans l'application.

Grossesse et allaitement

Grossesse
Unknown Pregnancy reports are too few to show a risk. In mice, prolonged labour, maternal deaths at birth and lower pup survival occurred from about the maximum human dose. If used in pregnancy, mix with preservative-free water (the bacteriostatic diluent has benzyl alcohol).
Allaitement
Unknown No data on metreleptin in milk; natural leptin is a normal milk component. It is a protein, so little is expected to be absorbed by the infant. No LactMed record. US label: weigh breastfeeding against the mother's need.

Effets indésirables signalés

  • Headache
  • Hypoglycemia
  • Decreased weight
  • Abdominal pain
  • Arthralgia
  • Dizziness

Mises en garde encadrées

  • Neutralizing anti-metreleptin antibodies have been identified; they could inhibit endogenous leptin action and/or cause loss of efficacy. Severe infection and/or worsening metabolic control have been reported. Test for neutralizing antibodies in patients with severe infections or suspected loss of efficacy.

Contre-indications

  • General obesity not associated with congenital leptin deficiency.
  • Prior severe hypersensitivity reactions to metreleptin or any product component.

Précautions particulières

  • Neutralizing antibodies may inhibit endogenous leptin and cause loss of efficacy; test if severe infections or loss of efficacy occur.
  • T-cell lymphoma: consider benefits and risks in patients with acquired generalized lipodystrophy or significant hematologic abnormalities.
  • Hypoglycemia with concomitant insulin or insulin secretagogues; monitor blood glucose.
