Mécanisme d'action
Sepiapterin is a precursor of the enzymatic co-factor tetrahydrobiopterin (BH4), which activates phenylalanine hydroxylase (PAH).
Posologie adulte habituelle (résumé de la notice)
Sepiapterin-responsive PKU (HPA), age 2 years and older: 60 mg/kg orally once daily with food; 60 mg/kg is the maximum daily dose for all patients
Grossesse et allaitement
- Grossesse
- Unknown Data are insufficient; no harm in rats or rabbits at up to 9 and 6 times human exposure. High maternal phenylalanine is itself harmful to the fetus (birth defects, microcephaly), so keep levels controlled before and during pregnancy.
- Allaitement
- Safe No data on milk levels or infant effects. US label: weigh benefits. LactMed: if the mother needs it, it is not a reason to stop breastfeeding, but an alternative may be preferred, especially with a newborn or preterm infant.
Effets indésirables signalés
- Diarrhea
- Headache
- Abdominal pain
- Hypophenylalaninemia
- Feces discoloration
- Oropharyngeal pain
Précautions particulières
- Increased bleeding risk (hematomas, prolonged bleeding, heavy menstrual bleeding); consider interruption in active bleeding.
- Hypophenylalaninemia in some pediatric patients; monitor blood Phe, with frequent monitoring in children.
- Levodopa co-administration: seizures, over-stimulation, irritability possible; monitor neurologic status.
Informez votre pharmacien ou votre médecin en cas de grossesse ou d'allaitement, de maladie rénale ou hépatique, d'allergies, ainsi que de tout autre médicament ou complément que vous prenez.
