Indications

Mécanisme d'action

Thyrotropin alfa binds TSH receptors on normal thyroid epithelial cells or well-differentiated thyroid cancer tissue, stimulating iodine uptake and organification and synthesis and secretion of thyroglobulin, T3 and T4. This increases radioiodine uptake for scan detection or radioiodine killing of thyroid cells.

Posologie adulte habituelle (résumé de la notice)

Adjunctive diagnostic tool or adjunct for thyroid remnant ablation in well-differentiated thyroid cancer: 0.9 mg intramuscular injection to the buttock, followed by a second 0.9 mg intramuscular injection 24 hours later; intramuscular only, not intravenous

Résumé de la notice pour la première ligne de posologie uniquement ; les sections complètes sur la posologie figurent dans l'application.

Grossesse et allaitement

Grossesse
Unsafe Used with radioiodine, it is contraindicated in pregnancy (UK label) because the fetus would be exposed to radiation, which can cause permanent thyroid damage. Data on thyrotropin alfa alone are too few to assess risk; no animal studies. Exclude pregnancy before use.
Allaitement
Unsafe With radioiodine: contraindicated (therapy) or stop breastfeeding after the dose (diagnosis), because of radiation to the infant and breast. Alone: no milk data; UK label says do not use while breastfeeding; US label: weigh benefits. Large protein, unlikely to reach the infant.

Effets indésirables signalés

  • Nausea
  • Headache
  • Fatigue
  • Vomiting
  • Dizziness
  • Asthenia

Contre-indications

  • When given with radioiodine, the contraindications to radioiodine also apply to the combination regimen.
  • Combination with radioiodine is contraindicated in pregnancy.
  • Concomitant use with therapeutic radioiodine is contraindicated in lactating women.

Précautions particulières

  • Hyperthyroidism: transient significant rise in thyroid hormone in patients with substantial thyroid tissue in situ or functional metastases; deaths reported. Elderly and heart disease patients are at risk; consider hospitalization and observation.
  • Stroke reported within 72 hours of administration, mostly in young women taking oral contraceptives or with other stroke risk factors. Ensure patients are well hydrated beforehand.
  • Sudden rapid painful tumor enlargement may occur (hemiplegia, hemiparesis, vision loss, laryngeal edema, respiratory distress). Consider glucocorticoid pretreatment.

Informez votre pharmacien ou votre médecin en cas de grossesse ou d'allaitement, de maladie rénale ou hépatique, d'allergies, ainsi que de tout autre médicament ou complément que vous prenez.
