Clemastine competes with histamine for the histamine H1 receptor, reversible competitive inhibitor.

- Unknown Limited human data with no clear link to birth defects. Loratadine, cetirizine or chlorphenamine have more pregnancy data and are preferred.

- Safe US label: do not use while nursing (antihistamines carry higher risk for infants). One 10-week-old had drowsiness, irritability, poor feeding and high-pitched crying (mother also on two antiseizure drugs). LactMed: small occasional or single bedtime doses are acceptable; prefer a non-sedating drug.
