Кратко
Tadalafil markedly potentiates the hypotensive effect of nitrates, which can cause severe, potentially life-threatening hypotension, syncope or myocardial ischaemia. The effect persists for a prolonged period because of tadalafil's long half-life. Nitrates in any form are contraindicated with tadalafil.

Isosorbide mononitrate and Tadalafil are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Nitrates increase cGMP by nitric oxide release while tadalafil inhibits PDE5-mediated cGMP breakdown, causing excessive vasodilation.
