Unknown Limited human data; belatacept crosses the placenta in animals, and in rats pup deaths followed infections in the mothers. It is given with mycophenolate, which is contraindicated in pregnancy. Plan pregnancy with the transplant team; registry available.

Unknown No human milk data; present in rat milk. Large protein, so oral infant absorption is likely low. Weigh benefits; note co-prescribed mycophenolate is not recommended in breastfeeding.

  • Increased risk of PTLD, predominantly CNS; use only in EBV seropositive patients, not in EBV seronegative or unknown serostatus. Only physicians experienced in immunosuppression and kidney transplant management should prescribe. Immunosuppression increases infection and malignancy risk. Not recommended in liver transplant due to increased graft loss and death.

  • PTLD risk, predominantly CNS; monitor for new or worsening neurological, cognitive or behavioral signs; check EBV serology before starting.
  • Increased risk of malignancies, including skin cancer; limit sun and UV exposure.
  • PML risk; consider in patients with new neurological, cognitive or behavioral symptoms; consider reducing immunosuppression if diagnosed.
