Safe Large registry data show no rise in birth defects. Long used to prevent mother-to-child HIV transmission, including IV during labour in some women. Transient raised lactate has been reported in exposed infants, without proven harm. Continue as prescribed in an HIV regimen.

Safe LactMed: well studied; low milk levels, most breastfed infants have undetectable blood levels, but some developed anaemia. US label: do not breastfeed (HIV). US/WHO guidance: with a sustained undetectable viral load, breastfeeding can be supported after counselling; otherwise formula or donor milk.

  • Risk of severe anemia and bone marrow depression; use with caution in patients with bone marrow compromise; hemoglobin reduction may occur 2-4 weeks
  • Myopathy and myositis with pathological changes, similar to that produced by HIV-1 disease,associated with prolonged use of therapy
  • Exacerbation of anemia due to ribavirin reported when zidovudine is part of HIV regimen; coadministration of ribavirin and zidovudine is not advised
