Isocarboxazid irreversibly inhibits monoamine oxidase, so serotonin is not broken down, and nortriptyline is a tricyclic antidepressant that blocks noradrenaline and serotonin reuptake. Together they can precipitate serotonin syndrome with hyperthermia, agitation, clonus and autonomic instability, which can be fatal.

Isocarboxazid and Nortriptyline are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
