Summary
Salicylates at anti-inflammatory doses can reduce renal prostaglandin synthesis, which may blunt the diuretic and antihypertensive effects of amiloride and reduce renal function. Together with amiloride's potassium-sparing action, this raises the risk of hyperkalemia and acute kidney injury, especially in volume-depleted or older patients.
What this interaction means in plain words
Amiloride and Choline magnesium trisalicylate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Salicylate-mediated prostaglandin inhibition lowers renal blood flow and potassium excretion, adding to amiloride's ENaC-mediated potassium retention.
What to do
Monitor serum potassium, creatinine and blood pressure, and ensure adequate hydration. Use the lowest effective salicylate dose or consider an alternative analgesic in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
