Summary

Amiloride and thiazide-type diuretics are frequently used together because amiloride counteracts potassium loss from the thiazide. Their effects on potassium are opposing, but electrolyte disturbances such as hyponatremia or hyperkalemia can still occur, mainly in renal impairment.

What this interaction means in plain words

Amiloride and Cyclopenthiazide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: High

Why it happens

Amiloride blocks collecting-duct ENaC and conserves potassium, while cyclopenthiazide blocks the distal tubule sodium-chloride cotransporter and increases potassium excretion.

What to do

Monitor serum potassium, sodium and renal function periodically, especially after dose changes or in older patients.

What to ask your pharmacist

  • Is there anything I should notice while taking both?
  • Do I need to tell my doctor at the next visit?
