Summary

NSAIDs such as fenbufen can reduce renal prostaglandin synthesis, leading to sodium retention, reduced diuretic effect and impaired renal function. Combined with amiloride, this increases the risk of hyperkalemia and acute kidney injury, especially in older or dehydrated patients.

What this interaction means in plain words

Amiloride and Fenbufen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

NSAID inhibition of prostaglandins suppresses renin-aldosterone signaling and renal perfusion, adding to amiloride's potassium retention.

What to do

Monitor serum potassium, creatinine and blood pressure, and keep the patient hydrated. Prefer the shortest NSAID course or an alternative analgesic in high-risk patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
