Summary
Amiloride is commonly paired with thiazide diuretics to offset potassium loss and improve blood pressure control. Their potassium effects oppose each other, but hyponatremia, volume depletion or hyperkalemia can occur, particularly with renal impairment.
What this interaction means in plain words
Amiloride and Methyclothiazide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Methyclothiazide blocks the distal sodium-chloride cotransporter and increases potassium excretion, while amiloride blocks ENaC and conserves potassium.
What to do
Check serum potassium, sodium and renal function periodically, particularly after dose changes.
What to ask your pharmacist
- Is there anything I should notice while taking both?
- Do I need to tell my doctor at the next visit?
