Summary
Parecoxib is converted to valdecoxib, a COX-2 inhibitor that can reduce renal prostaglandin synthesis, causing sodium retention and reduced renal perfusion. With amiloride, this raises the risk of hyperkalemia and acute kidney injury and may reduce diuretic efficacy.
What this interaction means in plain words
Amiloride and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
COX-2 inhibition lowers renal prostaglandins and aldosterone-related potassium excretion, adding to amiloride's potassium retention.
What to do
Monitor serum potassium, creatinine and blood pressure, ensure adequate hydration, and use the shortest effective course.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
