Summary
Combining an angiotensin receptor blocker with a thiazide diuretic is a common and effective antihypertensive strategy. The two drugs have additive blood pressure lowering, and the thiazide's potassium loss is partly offset by olmesartan. Excess hypotension, volume depletion or electrolyte disturbance can occur, especially at treatment start.
What this interaction means in plain words
Amlodipine-olmesartan and Bendroflumethiazide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Additive pharmacodynamic blood pressure lowering, with opposing effects on potassium: ARB reduces aldosterone-mediated potassium loss while thiazide increases renal potassium excretion.
What to do
Check blood pressure, serum potassium and sodium and renal function after starting or changing doses. Reduce the dose of one agent if symptomatic hypotension develops.
What to ask your pharmacist
- Is there anything I should notice while taking both?
- Do I need to tell my doctor at the next visit?
