Summary
Ketorolac is absorbed systemically after intranasal use and, like other NSAIDs, can blunt the blood pressure effect of olmesartan. Together they raise the risk of acute kidney injury and hyperkalemia, particularly in elderly or volume-depleted patients.
What this interaction means in plain words
Amlodipine-olmesartan and Ketorolac intranasal can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins combined with angiotensin receptor blockade reduces glomerular filtration and potassium excretion.
What to do
Avoid or limit ketorolac to a very short course, ensure adequate hydration, and monitor blood pressure, creatinine and potassium. Choose a non-NSAID analgesic where possible.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
