Summary

Rifampin strongly induces CYP3A4 and can markedly lower amlodipine concentrations, leading to loss of blood pressure control. Olmesartan is not meaningfully metabolized by CYP enzymes, so the effect mainly concerns the amlodipine component.

What this interaction means in plain words

Amlodipine-olmesartan and Rifampin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Induction of hepatic and intestinal CYP3A4 by rifampin increases amlodipine clearance.

What to do

Monitor blood pressure when rifampin is started or stopped and increase the antihypertensive dose or add another agent if control is lost. Reassess the dose after rifampin is discontinued because amlodipine levels will rise again.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
