Summary
Bowel preparations can cause fluid loss and electrolyte shifts, and in combination with an angiotensin receptor blocker this raises the risk of hypotension and acute kidney injury. Hypermagnesemia may add to calcium channel blocker effects such as hypotension and weakness.
What this interaction means in plain words
Amlodipine-olmesartan and Sodium sulfate / ?magnesium sulfate / potassium chloride can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Volume depletion and electrolyte disturbance from the laxative combine with additive vasodilation and reduced renal autoregulation from olmesartan.
What to do
Ensure adequate hydration during bowel preparation and consider withholding olmesartan on the day of the preparation in patients at risk. Check renal function and electrolytes in high-risk patients.
