Summary
Donepezil has a known risk of QT prolongation and torsades de pointes, and amoxapine, a tricyclic-related antidepressant, can also prolong the QT interval. Combined use may raise arrhythmia risk, especially with electrolyte disturbance or cardiac disease. Amoxapine's anticholinergic activity may also oppose donepezil's cholinergic benefit.
What this interaction means in plain words
Amoxapine and Donepezil can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive QT prolongation, plus pharmacodynamic opposition of cholinesterase inhibition by antimuscarinic effects.
What to do
Avoid the combination where possible; if used, obtain a baseline ECG, correct potassium and magnesium, and monitor the QTc. Consider an antidepressant with less QT and anticholinergic liability.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
