Arformoterol and chlorthalidone each can lower serum potassium, and combined use may produce clinically significant hypokalaemia with possible ECG changes or arrhythmia. Atenolol may also reduce the bronchodilator effect of arformoterol.
What this interaction means in plain words
Arformoterol and Atenolol-chlorthalidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Arformoterol belongs to Respiratory; Atenolol-chlorthalidone belongs to Cardiovascular.
Why it happens
Beta-2 agonist driven intracellular potassium shift adds to thiazide-induced renal potassium loss, and beta blockade antagonises the beta-2 agonist.
What to do
Check serum potassium and magnesium at baseline and periodically, and correct deficits. Monitor respiratory control given the beta blocker.
