Summary

Systemic corticosteroids can raise the risk of bleeding through effects on vascular fragility and gastric mucosa, and may rarely alter coagulation. Combined with a low molecular weight heparin, the overall bleeding risk may be somewhat higher.

What this interaction means in plain words

Bemiparin and Hydrocortisone have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Pharmacodynamic: corticosteroid effects on vascular integrity and gastric mucosa add to the anticoagulant effect of heparin; the effect on coagulability is inconsistent.

What to do

Watch for signs of bleeding, especially gastrointestinal, during prolonged or high-dose corticosteroid use with bemiparin. Monitor anti-Xa activity or platelet count only if clinically indicated; INR is not a useful marker for this heparin.
