Summary

Epinephrine shifts potassium into cells through beta2 stimulation, and thiazide diuretics cause renal potassium loss. Together they can produce additive hypokalemia, which raises the risk of ventricular arrhythmias, particularly with systemic or repeated epinephrine dosing.

What this interaction means in plain words

Bendroflumethiazide and Epinephrine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Beta2-mediated cellular potassium uptake combined with thiazide-induced renal potassium wasting.

What to do

Check serum potassium and magnesium and replace deficits when epinephrine is used systemically in patients taking thiazides. Monitor ECG in patients with cardiac disease.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
