Overview
Additive glucose-lowering and possible ↑ metformin exposure may increase risk of hypoglycemia and GI intolerance; theoretical increased risk of lactic acidosis in predisposed patients.
Mechanism of action
Pharmacodynamic additivity (antihyperglycemic). Berberine may inhibit OCT2/MATE transporters, reducing metformin clearance and increasing concentrations.
Clinical management
Monitor fasting and postprandial glucose and for hypoglycemia; consider reducing metformin dose if glycemia falls or adverse effects occur. Assess renal function (eGFR) and counsel on lactic acidosis warning signs.
