Summary

Regular use of analgesic-dose salicylates may blunt the antihypertensive effect of bisoprolol and can impair renal function and cause fluid retention. Low-dose aspirin is much less likely to be significant.

What this interaction means in plain words

Bisoprolol and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Inhibition of prostaglandin synthesis leads to sodium and water retention and reduced vasodilation, opposing the beta-blocker's blood pressure effect.

What to do

Monitor blood pressure if salicylates are taken regularly at analgesic doses. Prefer paracetamol or the lowest effective salicylate dose for a short duration.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
