Summary
Bretylium commonly causes hypotension through adrenergic neuron blockade, and the thiazide and angiotensin receptor blocker in this combination product also lower blood pressure. Together they may produce additive or excessive hypotension, particularly on standing.
What this interaction means in plain words
Bretylium and Hydrochlorothiazide-olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive pharmacodynamic blood pressure lowering through sympathetic blockade, volume depletion and angiotensin receptor blockade.
What to do
Monitor blood pressure closely, including orthostatic readings, when bretylium is given. Reduce or hold the antihypertensive combination if hypotension develops.
