Bromazepam + Gabapentin

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Additive CNS depressant effects may lead to excessive sedation, dizziness, impaired psychomotor performance, and increased risk of respiratory depression, especially in older adults, those with respiratory disease, sleep apnea, or when combined with other CNS depressants (e.g., opioids, alcohol).

Mechanism of action

Pharmacodynamic: Benzodiazepine GABA-A positive allosteric modulation plus gabapentin’s α2δ calcium channel binding reduce neuronal excitability, producing additive/synergistic CNS and respiratory depression; no meaningful pharmacokinetic interaction expected.

Clinical management

Avoid combination if possible; if used, start at the lowest effective doses and titrate cautiously. Avoid other CNS depressants (including alcohol). Monitor for excessive sedation, confusion, falls, and signs of respiratory depression (slow or shallow breathing), particularly after dose changes and in high-risk patients. Consider dose reduction and enhanced monitoring in elderly or those with pulmonary disease; adjust gabapentin for renal impairment.
