Summary
Corticosteroids raise blood glucose and can reduce the effect of insulin. With systemically absorbed budesonide, such as oral or high-dose inhaled use, insulin requirements may increase in people with diabetes. Effects are smaller than with prednisolone-type agents because of budesonide's high first-pass metabolism.
What this interaction means in plain words
Budesonide and Insulin nph can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Glucocorticoid-induced insulin resistance and increased hepatic gluconeogenesis oppose the glucose-lowering effect of insulin.
What to do
Monitor blood glucose more frequently when starting or increasing budesonide and titrate insulin dose as needed. Reassess insulin dose when the corticosteroid is reduced or stopped.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
