Summary

NSAIDs including salicylates can impair renal function when combined with ACE inhibitors, especially in elderly or volume-depleted patients. They may also blunt the antihypertensive effect of captopril.

What this interaction means in plain words

Captopril and Magnesium salicylate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of prostaglandin synthesis reduces renal perfusion and counteracts ACE inhibitor effects on blood pressure and glomerular filtration.

What to do

Maintain hydration and monitor serum creatinine, potassium and blood pressure, particularly in older or volume-depleted patients. Prefer a non-NSAID analgesic such as paracetamol where possible.

What to ask your pharmacist

  • Should I take them at different times of day?
