Summary
Levodopa can cause orthostatic hypotension, and thiazide diuretics lower blood pressure through volume depletion. Together they may increase the risk of dizziness, falls and symptomatic hypotension, especially in older patients.
What this interaction means in plain words
Carbidopa-entacapone-levodopa and Cyclopenthiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive hypotensive effects: levodopa-related vasodilation and autonomic effects combined with diuretic-induced volume and sodium loss.
What to do
Check blood pressure including standing readings when starting or adjusting either drug, and reduce the diuretic dose if orthostatic symptoms develop. Maintain adequate hydration.
