Summary

Transdermal selegiline at antidepressant doses inhibits MAO-A and MAO-B non-selectively. With levodopa, accumulated catecholamines can precipitate hypertensive crisis.

What this interaction means in plain words

Carbidopa-levodopa and Selegiline transdermal are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: High

Why it happens

Non-selective MAO inhibition blocks catecholamine breakdown, potentiating levodopa-derived dopamine and noradrenaline.

What to do

Avoid the combination; use another antidepressant or antiparkinsonian regimen.
