Summary
Nonselective beta-blockade with carvedilol leaves the alpha-mediated vasoconstriction of epinephrine unopposed, which can cause marked hypertension with reflex bradycardia. The beta-mediated effects of epinephrine, such as bronchodilation and cardiac stimulation, are blunted, which can reduce its benefit in anaphylaxis.
What this interaction means in plain words
Carvedilol and Epinephrine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Carvedilol belongs to Cardiovascular; Epinephrine belongs to Endocrine.
Why it happens
Pharmacodynamic antagonism at beta receptors, leaving epinephrine's alpha-adrenergic effects unopposed.
What to do
Monitor blood pressure and heart rate closely when epinephrine is given to a patient on carvedilol. In anaphylaxis unresponsive to epinephrine, consider glucagon, and be prepared to treat severe hypertension or bradycardia.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
