Drug interactions
Carvedilol + Perindopril
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Additive antihypertensive effects may lead to symptomatic hypotension/orthostasis (dizziness, syncope), especially at initiation or with dose increases and in volume-depleted or elderly patients.
Mechanism of action
Pharmacodynamic: ACE inhibition lowers angiotensin II–mediated vasoconstriction; carvedilol blocks beta-adrenergic (and alpha1) receptors—combined effects reduce blood pressure.
Clinical management
Use low starting doses and titrate gradually; monitor blood pressure (including orthostatic) during initiation and titration; counsel patients on rising slowly; if symptomatic hypotension occurs, hold or reduce one agent and correct volume depletion; monitor serum creatinine and potassium within 1–2 weeks after starting or increasing perindopril and after significant regimen changes.
